Scheduling Your Appointment
Contact Us: Since everyone's issues and needs are different, we ask that you call our office at (941) 251-TREX (8739) to schedule your appointment.

New Patient Forms: We recommend completing the online intake forms that we email to you before your visit as this will save time at your appointment.

Rescheduling or Cancelling: As a courtesy to everyone, we require a 24 hour notice as a common courtesy in the event of needing to reschedule or cancel appointment.

Come Prepared
We are excited that you took the first step in your chiropractic journey! Please remember the following tips to ensure a successful first visit:
Arrive on Time: Being punctual to your appointment will ensure that we have sufficient time for your health history review, examination, and treatment.

Wear Appropriate Clothing: Wear clothing that is easy to move in and will allow access to area for treatment.

Bring Imaging Studies & Reports: This includes any x-rays, MRI, CT scans and lab work.
Discuss your health history and current concerns.
Perform a thorough physical examination that includes a spinal assessment, strength and range of motion tests.
Review any diagnostic imaging (x-rays, MRIs).
Explain findings and develop a personalized treatment plan designed specifically to your needs.
